Need Back-to-School Medication Paperwork?

Back to School Medication Forms!

Believe it or not, it’s almost time to send the kids back to school! (We’re in shock, too.) That means that back-to-school paperwork is almost due.

At Allergy & Asthma Care, we believe in making your life as simple as possible. If your child is a patient of ours, we’ll fill out the necessary medication paperwork at no charge to you.

It takes just 3 simple steps:

  1. Get forms from your child’s school. You’ll need a separate form for each medication, and fill out each of the forms completely.
  2. Fax the paperwork to us, or you can bring it into one of the offices. We’ll have one of our doctors sign the form.
  3. Then, we’ll fax the form(s) to the child’s school. Make sure the school’s fax number is on the form!
